CSE Program Forms
Our CSE Forms use DocuSign for information entry and approval signatures. When you select a form below that utilizes DocuSign, you will be able to assign recipients for signatures and complete the necessary form fields electronically.
Step 1: Choose a DocuSign enabled form below.
Step 2: Log into DocuSign using your gtAccount@gatech.edu credentials.
Step 3: In DocuSign, navigate to Shared Folders > _Students > COC > Computational Science and Engineering folder
Step 4: Click the USE button on the desired form.
Step 5: Using the DocuSign address book, complete the Recipients section of the form.
Step 6: Edit the "Email Subject" to include your first and last name [Last name, First name].
Step 7: Click SEND.
Step 8: You as the student will be the first recipient of the form in DocuSign. Complete the necessary fields and when finished, click FINISH.
The form will be routed automatically to other recipients to review and sign. Once competed, all recipients recieve the signed and completed form via email. You can check the progress of the form in your DocuSign account.

This form allows a student to register for a specified 8903 section. Due: By the end of Phase 1 registration |
