CSE Program Forms

Our CSE Forms use DocuSign for information entry and approval signatures. When you select a form below that utilizes DocuSign, you will be able to assign recipients for signatures and complete the necessary form fields electronically.

Step 1: Choose a DocuSign enabled form below.

Step 2: Log into DocuSign using your gtAccount@gatech.edu credentials.

Step 3: In DocuSign, navigate to Shared Folders > _Students > COC > Computational Science and Engineering folder

Step 4: Click the USE button on the desired form.

Step 5: Using the DocuSign address book, complete the Recipients section of the form.

Step 6: Edit the "Email Subject" to include your first and last name [Last name, First name].

Step 7: Click SEND.

Step 8: You as the student will be the first recipient of the form in DocuSign. Complete the necessary fields and when finished, click FINISH.

The form will be routed automatically to other recipients to review and sign. Once competed, all recipients recieve the signed and completed form via email. You can check the progress of the form in your DocuSign account.

Master's Forms

Computational Science and Engineering (CSE 8903) Permit Form

This form allows a student to register for a specified 8903 section.

Due: By the end of Phase 1 registration

List the proposed courses to satisfy the degree requirements.

Due: By end of 1st semester of enrollment

MS Thesis (CSE 7000) Permit Form

For approval to register for MS Thesis Hours

Due: One week prior to the beginning of Phase I registration

MS Thesis Committee Approval Form

To list the proposed members of the MS Thesis Committee

Due: Minimally one month prior to the MS defense

Frequency: any time changes made to form

Ph.D. Forms

Computational Science and Engineering

Special Problem Permit Form

This form allows a student to register for a specified 8903 section.

Due: By the end of Phase 1 registration.

CSE Ph.D. Advisor Selection and

Pre-Dissertation (CSE 8999) Permit Form

For designating or for changing advisors.

Due: Upon selection or modification of Advisor.

CSE PHD Minor Form

To show completion of the non-CS/CSE coursework that comprises the Institute’s PhD Minor component.  Minimum 9 credit hours
CSE Written Qualifying Exam Registration

To select the areas for the written Qualifying exam
Due: Minimally 4 weeks prior to Quals exam
Frequency: any time changes made to form

CSE Oral Qualifying Exam Committee Approval Form

To list the proposed PhD Quals committee members for the oral Qualifying exam

Due: Within 2 weeks of being notified of passing the written Qualifying exams

CSE PhD Qualifier & 9000 Permit

 

To show completion of the CS/E PhD Qualifier Exam and to obtain a permit for CS/E 9000.

Due: two weeks after notification of Exam date

Frequency: any time changes made to form

PhD Proposed Program of Study Form

To list proposed courses for completing the PhD degree

Due: By end of 1st semester of enrollment

Frequency: Any time changes made to form

PhD Thesis Committee Approval Form

To list the proposed members of the PhD Thesis Committee

Due: Minimally one month prior to the PhD proposal date

Frequency: any time changes made to form

PhD Admission to PhD Candidacy

To officially declare PhD candidacy

Due: After completing all core requirements (courses, qualifier, committee selection, program of study, thesis topic)
